Job Summary:

Our organization is seeking a dedicated Care Coordinator to oversee the delivery of care services to our clients. The successful candidate will have extensive experience in coordinating, managing, and administering care services.

Key Responsibilities:

  • Manage a caseload of 30-40 clients.
  • Serve as the primary point of contact for clients regarding matters such as rostering, budgets, purchases, and more.
  • Provide guidance and technical support to care staff.
  • Assume leadership roles within the organization and serve as a role model.
  • For experienced nurses, provide nursing support to clients.
  • Advocate on behalf of clients for all matters.

Requirements:

  1. Certification in aged or disability care, or registration as a registered nurse with current AHPRA registration, or enrollment as an enrolled nurse with current AHPRA registration.
  2. Experience working in aged care, disability, or mental health settings.
  3. Understanding of consumer-directed care and related standards.
  4. Current CPR certification, first aid training, and a national police check.
  5. Working with children Blue Card or willingness to obtain.
  6. Valid Australian driver's license.
  7. High level of computer literacy and understanding of technology.
  8. Understanding of clinical governance and continuous improvement principles.
  9. Positive attitude, ability to work well in a team environment, and excellent customer service skills.

Preferred Skills:

  • Previous experience with rostering is highly desirable.
  • Proficiency in Microsoft Word and Excel.
  • Outgoing personality with high energy levels, good communication skills, and a personable telephone manner.
  • Excellent networking and consultation skills to develop new referral sources and relationships with external organizations.
