About This Role

This role offers a unique opportunity to make a meaningful impact in the lives of others. As a Home Care Team Leader, you will be responsible for coordinating services and managing care plans to support both clients and care workers.

Your day-to-day tasks may vary, but they will always involve working closely with your team to ensure that everyone feels confident, equipped, and valued in their roles. You will also be required to mentor staff, review services, solve rostering puzzles, and work with families and health providers to keep things running smoothly.

This is a varied and rewarding role that requires strong organizational skills, calmness under pressure, and a genuine passion for helping others. If you are organized, compassionate, and enjoy working as part of a collaborative team, then this could be the perfect opportunity for you.

Key Responsibilities
  • Coordinate services and manage care plans to support clients and care workers
  • Mentor staff and provide guidance on best practices
  • Review services and make recommendations for improvement
  • Solve rostering puzzles and optimize team schedules
  • Work with families and health providers to ensure smooth operations
Requirements

To be successful in this role, you will need:

  • A Certificate III in Individual Support (Ageing) or equivalent
  • Completed HLTHPS006, HLTAAP001 & CHCCCS031
  • First Aid & CPR, driver's licence, insured vehicle
  • Experience leading a home care or community care team

  • Intermediate computer skills
