Case Worker Role

We are seeking a dedicated and compassionate professional to join our team as a Case Worker.

Key Responsibilities:

  • Develop case plans with clients, provide ongoing support, and conduct regular reviews to achieve the case plan objectives.
  • Maintain accurate and comprehensive client records and case notes in accordance with service policy requirements.
  • Engage with individuals experiencing homelessness or at risk of becoming homeless, providing essential guidance and support services.
  • Collaborate with other key stakeholders and services to create comprehensive support networks for clients, promoting social inclusion and community connections.
  • Occasionally train and support student placements at the service, fostering learning and growth opportunities.
  • Refer clients to relevant external services and connect them with life skill and educational programs, enhancing their overall well-being.
  • Ensure a safe and welcoming environment for clients accessing the service, promoting dignity and respect at all times.

Requirements:

  • Minimum TAFE Certificate IV or current tertiary study in a welfare field, or extensive experience in the sector.
  • Excellent written and oral communication skills, with proficiency in computer software applications.
  • Ability to work independently and as part of a collaborative team, prioritizing a strong focus on customer-centric outcomes.
  • Previous experience supporting marginalized and vulnerable individuals, preferably in a homelessness/housing setting context.
  • Knowledge of issues related to homelessness, including family breakdown, mental illness, substance abuse, and addiction.
  • Awareness of local community support services for homeless clients, including government agencies and NGOs, and the capacity to build strong relationships with these providers.
