Job Description

We are seeking a dedicated Care Coordinator to provide intensive support and outreach to individuals who are homeless or at risk of homelessness. The goal is to ensure they have access to safe accommodation and prevent their return to the homelessness service system.

The crisis accommodation residential program provides comprehensive support to clients, addressing presenting needs, risks, and vulnerabilities. Responsibilities include assessing client referrals, providing regular contacts with safety plans developed, creating personalized action plans in collaboration with clients, and assisting them in accessing support services.

You will work closely with clients to understand their needs and develop tailored plans to address their challenges. This role involves engaging with a range of stakeholders, including community organizations, government agencies, and other service providers.

Required Skills and Qualifications

To be successful in this role, you will require:

  • A tertiary qualification in Social Work, Social Welfare, or related field
  • Demonstrated experience working with individuals experiencing homelessness who display challenging behaviors
  • Knowledge of service systems, particularly youth, housing, and homelessness, Child Protection, AOD, Family Services, and Mental Health
  • Skill in engaging homeless persons
  • A valid Employee Working with Children's Check and a current Victorian Driver's Licence
