Job Opportunity:
We are seeking a skilled Patient Care Coordinator to join our dialysis team. The successful candidate will be responsible for coordinating outpatient dialysis services and treatments within assigned hospitals/facilities.
Key Responsibilities:
- Coordinate and schedule outpatient dialysis treatments and staff to ensure coverage and efficient utilization.
- Ensure treatments are performed according to policies, procedures, and regulations, with accurate documentation.
- Provide guidance and support to outpatient care staff; collaborate on staffing, hiring, and disciplinary actions.
- Ensure provision of dialysis services such as CRRT, hemoperfusion, peritoneal dialysis, and plasmapheresis.
- Participate in ongoing education on dialysis developments.
- Contribute to Quality Assessment and Improvement programs, maintain documentation, and report progress.
- Conduct outpatient staff meetings and ensure proper staff training and ongoing education.
- Assess patient needs, communicate with physicians, and implement care plan changes.
- Perform outpatient treatments and oversee dialysis-related medical and emergency functions.
- Monitor patient conditions and administer medications as ordered.
- Serve as a resource and subject matter expert for staff.
- Coordinate maintenance, infection control, and equipment management with technical staff.
- Operate dialysis and emergency equipment safely.
- Act as liaison among clinical staff, management, and facilities.
- Oversee billing, payroll, purchase orders, and reporting activities.
Requirements:
- Graduate of an accredited School of Nursing (R.N.)
- Current state licensure
- At least 9 months nursing experience plus 3 months dialysis care experience
- Leadership skills, quality improvement, team building, and communication skills required
