This position requires an individual with exceptional interpersonal and communication skills, who can work effectively as part of a multidisciplinary team to deliver high-quality patient care.
About the PositionThe successful applicant will be responsible for supporting patients in their homes, providing assistance with personal care, transport, therapy programs, and equipment setup.
- Key Responsibilities:
- Demonstrate empathy and understanding towards patients, families, and carers, promoting independence and dignity at all times.
- Deliver delegated interventions under the guidance of Allied Health Professionals, ensuring seamless continuity of care.
- Contribute to patient records, maintaining accurate and comprehensive documentation.
- Collaborate with healthcare professionals to achieve optimal patient outcomes, adhering to organizational standards.
