Job Title: Patient Care Specialist

">About the Role

We are seeking a highly motivated and compassionate Patient Care Specialist to join our team. In this role, you will provide exceptional support services to customers with diverse needs, working closely with Allied Health Clinicians to deliver high-quality interventions.

You will be responsible for assisting in the delivery of individual and group intervention programs, monitoring customer treatment plans, and collaborating with clinicians to ensure progress and identify areas for review.

This is an exciting opportunity to make a positive impact on people's lives, providing instruction, supervision, and education to customers, support workers, and other carers as required. You will also assist in program preparation and supervise participants under the guidance of Allied Health Clinicians.

Key Responsibilities
  • Assist in the delivery of high-quality individual and group intervention programs.
  • Regularly monitor customer treatment plans and collaborate with clinicians to notify of progress, concerns, and need for review.
  • Provide support services to customers presenting with a wide range of needs, including aged care, disability, health and wellbeing, return to work, and rehabilitation, in collaboration with Allied Health Clinicians.
  • Provide instruction, supervision, and education to customers, support workers, and other carers as required under the direction of an Allied Health Clinician.
  • Assist in program preparation and supervise participants under the guidance of Allied Health Clinicians.
Requirements

To succeed in this role, you will require:

  • A strong understanding of customer-focused support services.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively with healthcare professionals and customers.
  • Strong organizational and time management skills.
  • Diploma or certificate in a relevant field (e.g., allied health, nursing, social work).
