Key Responsibilities

  • Support clients with complex needs to address homelessness or maintain tenancy under a case management framework;">
  • Complete intake and assessments of new clients;">
  • Promote independent living skills and choice;">
  • Collect and report client data as required through the use of an electronic database;">
  • Liaise with other community and government agencies and Real Estate Agencies as part of your case management;">
  • Ensure service delivery meets all legislative guidelines and internal procedures;">
  • Travel across the Fraser Coast region;">

What We Need from You

">

To be successful in this role, you will need:

">
  • ">
  • A qualification in Community Services, Mental Health, or Social Housing, or experience in the community sector;">
  • Experience in accommodation services or real estate would be highly regarded;">
  • Knowledge and understanding of the Housing framework;">
  • A current QLD Driver's licence;">
  • A National Police Certificate;">
  • A Working with Children Blue Card and NDIS Worker Screening Clearance;">
  • Strong communication skills, both written and verbal;">
  • Excellent time management, prioritisation, and problem-solving skills;">
  • A commitment to a person-centred approach of service provision;">

Salary and Benefits

You will be paid an hourly rate of $32.36 per hour, commencing at $63,953 per annum, depending on your qualifications and/or previous experience. In addition to your salary, Centacare Brisbane offers:
