">About the Role:
">We are seeking a highly skilled and experienced Mental Health Clinician (Clinical Care Coordinator) to join our team in Hervey Bay. As a key member of our multidisciplinary team, you will play a critical role in providing essential services to clients with severe mental health conditions throughout the Country to Coast Queensland region.
">Your Responsibilities Will Include:
">- Clinical Assessments: Conduct thorough clinical assessments and develop tailored treatment plans to meet the unique needs of each client.">
- Psycho-Social Interventions: Deliver evidence-based psychosocial interventions to support clients in achieving their recovery goals.">
- Clinical Caseload Management: Manage a clinical caseload and conduct risk assessments with management plans to ensure the safety and well-being of clients.">
- Data Collection and Monitoring: Engage in data collection and contribute to monitoring and evaluation processes to inform service improvement.">
- Multidisciplinary Team Collaboration: Collaborate with multidisciplinary teams and external stakeholders to ensure seamless coordination of care.">
- Care Planning and Review: Facilitate care planning and review processes to ensure that clients receive comprehensive and coordinated care.">

Requirements:
">- Qualifications: Current annual practicing certificate from AHPRA or registered with AASW.">
- Education: Tertiary qualification in psychology, social work, mental health nursing, or a relevant discipline.">
- Experience: Experience working with complex mental health conditions, particularly in cross-cultural settings.">
- Skills: Strong problem-solving and analytical skills, excellent organisational and time management skills, and proficiency with Microsoft Office and software.">
- Documentation: Driver's Licence, Police check, and Working with Children Check (blue card).">
Estimated Salary: $80,000 - $110,000 per annum, depending on experience.
