Patient Care Coordinator

As a Registered Nurse at our Mental Health Rehabilitation Unit, you will play a vital part in the delivery of high-quality patient care to individuals with mental health needs.
Your primary responsibility will be to coordinate and deliver nursing care that is consistent with our policies, procedures, and standards.
You will work collaboratively with other healthcare professionals to provide comprehensive care that meets the physical, emotional, and psychological needs of our patients.
Key responsibilities- Coordinate and deliver high-quality nursing care to patients/clients of the Mental Health Rehabilitation Unit.
- Work collaboratively with other healthcare professionals to provide comprehensive care.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Monitor and evaluate patient progress, making adjustments to care plans as needed.
- Tertiary qualifications in Nursing.
- Current registration as a Registered Nurse with the Nursing & Midwifery Board of Australia.
- Demonstrated high level written and verbal communication skills.
- Demonstrated ability to work effectively in a team environment.
- Basic computer skills and proficiency in using health information systems.
