About the Role
- This is a full-time opportunity available working Monday to Friday
- To service clients in Lake Macquarie East
The successful candidate will be responsible for providing high-quality, person-centred care to clients in their homes, promoting independence, dignity, and well-being. Key responsibilities include:
- Providing health promotion and education to clients and carers
- Promoting and delivering wellness programs in person and virtually
- Monitoring and promoting health outcomes via Telehealth
- Providing direct care services across a range of different funding programs, including wound care, diabetes care, medication management, stoma care, and catheter care
- Assessing, developing, and evaluating nursing care for all clients through a collaborative approach
- Working with clients to develop and achieve client care plans that are goal-driven and client-centred
- Assisting clients to become independent with managing health needs through a Wellness and Reablement approach
To be successful in this role, you will need to have:
- A Division One Registered Nurse qualification with the Australian Health Practitioners' Regulation Authority (AHPRA)
- Experience working in community health care, preferably with a focus on primary health care and consumer-directed care
- Excellent communication and interpersonal skills, with the ability to work effectively with clients, carers, and other healthcare professionals
- A strong understanding of community health principles, including health promotion, disease prevention, and health education
- Proficiency in the use of electronic technology, including smartphones, tablets, laptops, email, internet, and client data management systems
- Ability to work independently and as part of a team, with a focus on collaboration and mutual respect
- Current driver's license and ability to travel for client care
