This role is part of a statewide, high-intensity, transitional rehabilitation program offering clinical and psychosocial support in a recovery-focused framework. The successful candidate will work with individuals aged 16 and over to deliver individualised recovery support based on their goals.
Key Responsibilities:- Assist in delivering recovery support for consumers with a mental illness.
- Monitor and review consumer recovery plans with Clinical Leads, carers/family members/clinicians and community organisations.
- Promote positive self-care strategies, enhance social and daily living skills, increase vocational capacity and support community integration.
- Complete risk assessments and safety planning activities as required for consumers engaged with the service.
