Pastoral Care Coordinator Role

As a spiritual care leader, you will provide crucial support to clients and their families in various settings across the Northshore. This role involves visiting clients in their homes, requiring reliable transportation with mileage reimbursement.

Main responsibilities include:

  • Offering compassionate care and guidance to clients, relevant family members and staff
  • Serving as a liaison for internal and external support services
  • Mentoring colleagues on delivering tailored spiritual support to meet clients' diverse needs
  • Participating in multidisciplinary team meetings to contribute to comprehensive care plans
About You:
  • Alignment with HammondCare's Christian Mission and core values
  • In-depth understanding of palliative care for individuals living at home with life-limiting illness and its impact on caregivers
  • Demonstrated experience in palliative care is preferred but not essential
