Adult Social Care Professional

We are seeking an experienced Adult Social Care Professional to join our team in Knowsley.

About the Role:

  • This is an exciting opportunity for a qualified social worker to work in a busy Adults Community/Locality team, providing high-quality care and support to adults in need.
  • The post holder will be required to complete comprehensive assessments of adult needs, develop and implement effective support plans, and provide guidance on commissioning services as needed.
  • You will also be responsible for risk assessment and management, including attending Multi-Agency Risk Assessment Conferences (MARACs), and ensuring that safeguarding practices are upheld at all times.

Main Responsibilities:

  • Conduct thorough assessments of adult needs, identifying areas of vulnerability and developing personalized support plans.
  • Work closely with service users, their families, and other stakeholders to ensure that care and support needs are met.
  • Provide expert advice and guidance on commissioning services, including Continuing Healthcare – Decision Support Tool (CHC-DST) assessments.
  • Participate in MARACs and other multi-agency meetings to discuss risk management strategies and ensure effective collaboration between agencies.

Requirements:

  • A degree in Social Work or a related field, with a recognized qualification in adult social care.
  • Significant experience working in adult social care, preferably in a Locality or Community Team environment.
  • Proven ability to assess complex adult needs, develop effective support plans, and manage risk.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
  • A full driving license and willingness to travel within Knowsley and surrounding areas.
