The successful candidate will play a critical role in achieving safe, coordinated, and streamlined transitions for clients from acute mental health inpatient settings to the community.
With a focus on maximizing connections with relevant supports and minimizing early readmissions, this position requires strong clinical skills, effective communication, and collaboration with other healthcare professionals.
Key Responsibilities:- Achieve safe and effective transitions for clients
- Provide clinical input to develop strong connections with community supports
- Build partnerships with clinicians to achieve shared goals
To be successful in this role, you will need:
- An approved degree in Psychology or Occupational Therapy, or an approved degree in Social Work with eligibility for membership of the Australian Association of Social Workers
- Registration with AHPRA as a Psychologist, Occupational Therapist, or Social Worker
- A current Victorian Driver's License and ability to drive a work vehicle
