About the opportunity

The objectives of the PAST Clinician is to:

* Achieve safe, coordinated and streamlined transition for clients from an acute mental health inpatient setting to the community.

* Provide clinical input to maximize the likelihood of developing strong connections with relevant supports within the community and minimizing an early readmission (within 28 days).

* Build strong partnerships with other clinicians within and outside of Mercy Health who are working to achieve the same goal.

Qualifications:

* An approved degree in Psychology and registration with AHPRA as a Psychologist with endorsement as a Clinical Psychologist or

* An approved degree in Occupational Therapy and registration with AHPRA as an Occupational Therapist or

* An approved degree in Social Work and eligibility for membership of the Australian Association of Social Workers

* A current Victorian Driver’s License and ability to drive a work vehicle
