About the Role

We are seeking an experienced Clinical Care Coordinator to join our team at Southern Cross Care NSW & ACT. As a key member of our care team, you will play a vital role in supporting our Registered Nurses and Care staff in delivering high-quality clinical care to our residents.

Key Responsibilities
  • Care Planning and Coordination
    • Monitor and review care plans to ensure they are up-to-date and meet the individual needs of our residents.
    • Collaborate with the care team to develop and implement care plans that promote the health, well-being, and quality of life of our residents.
  • Clinical Advice and Support
    • Provide expert clinical advice and support to care staff to ensure the delivery of high-quality care to our residents.
    • Act as a liaison between healthcare professionals, including doctors, paramedics, and hospitals, to ensure seamless care coordination.
  • Quality Improvement and Compliance
    • Monitor and report on the implementation of care plans and quality of care provided to our residents.
    • Identify areas for quality improvement and implement strategies to address these areas.
  • Leadership and Teamwork
    • Lead by example and demonstrate a commitment to delivering high-quality care to our residents.
    • Collaborate with the care team to promote a positive and supportive work environment.
Requirements
  • 5 years' experience in caring for the aged or health-related area.
  • Registered Nurse experience.
  • Strong leadership and interpersonal skills.
  • Good written and verbal communication skills.
  • Ability to work collaboratively as part of a care team.
  • Numeracy skills and ability to maintain accurate records.
  • Sensitivity to and understanding of the needs of the elderly in a residential care setting.
Desirable
  • Sound understanding of relevant State and Commonwealth legislation as it applies to Aged Care Facilities.
