Key Responsibilities
  • Care Planning and Coordination
    • Monitor and review care plans to ensure they are up-to-date and meet the individual needs of our residents.
    • Collaborate with the care team to develop and implement care plans that promote independence, dignity, and quality of life.
  • Clinical Leadership
    • Provide clinical guidance and support to care staff to ensure the delivery of high-quality care.
    • Conduct regular audits to ensure compliance with regulatory requirements and industry standards.
  • Communication and Collaboration
    • Liaise with external healthcare professionals, including doctors and hospitals, to ensure seamless care transitions.
    • Communicate effectively with residents, families, and care staff to ensure open and transparent care planning.
  • Quality Improvement
    • Identify areas for quality improvement and implement strategies to address these.
    • Monitor and evaluate the effectiveness of care interventions and make recommendations for change.
Requirements
  • 5 years' experience in caring for the aged or health-related area.
  • Registered Nurse experience.
  • Strong leadership and communication skills.
  • Ability to work collaboratively as part of a multidisciplinary care team.
  • Numeracy skills and ability to maintain accurate records.
Desirable
  • Sound understanding of relevant State and Commonwealth legislation as it applies to Aged Care Facilities.
