Key Responsibilities- Care Planning and Coordination
- Monitor and review care plans to ensure they are up-to-date and meet the individual needs of our residents.
- Collaborate with the care team to develop and implement care plans that promote independence, dignity, and quality of life.
- Clinical Leadership
- Provide clinical guidance and support to care staff to ensure the delivery of high-quality care.
- Conduct regular audits to ensure compliance with regulatory requirements and industry standards.
- Communication and Collaboration
- Liaise with external healthcare professionals, including doctors and hospitals, to ensure seamless care transitions.
- Communicate effectively with residents, families, and care staff to ensure open and transparent care planning.
- Quality Improvement
- Identify areas for quality improvement and implement strategies to address these.
- Monitor and evaluate the effectiveness of care interventions and make recommendations for change.
- 5 years' experience in caring for the aged or health-related area.
- Registered Nurse experience.
- Strong leadership and communication skills.
- Ability to work collaboratively as part of a multidisciplinary care team.
- Numeracy skills and ability to maintain accurate records.
- Sound understanding of relevant State and Commonwealth legislation as it applies to Aged Care Facilities.
