Job Title: Lifestyle Facilitator

Mercy Community Aged Care Bethany is seeking a Lifestyle Facilitator to support the team in delivering meaningful activities to residents.

Key Responsibilities:
  • Identify individual resident needs with respect to capacity, skill and preferences related to their lifestyle activities
  • Implement individual & group activities, reporting on individual outcomes and participating in reviews of programs and outcomes
  • Develop a Monthly Activity Program with Facility Manager input
About You:
  • Certificate III in aged care (experienced in Lifestyle Program) or Cert IV Leisure & Lifestyle
  • Current First Aid certificate
  • Previous experience in a similar role, Aged Care experience beneficial
  • Queensland Provisional Green or above Driver License
