Role Overview
We are seeking a skilled and compassionate Clinical Care Coordinator to join our team in VIC. As a Clinical Care Coordinator, you will play a crucial role in managing client care and well-being, ensuring the safe provision of clinical care and compliance with regulations.
Key Responsibilities
- Conduct comprehensive clinical assessments and interventions in specialized areas such as wound management, continence management, medication management, dementia care support, and diabetes management and education.
- Manage a small case load, ensuring efficient and effective care coordination.
- Initiate service agreements with external providers as necessary.
- Demonstrate in-depth knowledge of industry regulations and best practices.
- Mentor and support direct care staff to ensure high-quality care delivery.
- Respond to clinical inquiries and work with health professionals to develop care plans.
- Monitor and review care plans within budget limitations.
- Advocate for consumers to ensure timely clinical care.
- Coordinate Peer Review Meetings and follow-ups.
- Network and build relationships with community resources to support client care.
- Ensure compliance with legislation, guidelines, and standards.
- Maintain consumer confidentiality and privacy.
- Participate in quality reporting activities to drive continuous improvement.
Requirements
- 3 to 5 years of experience as a Registered Nurse (RN).
- Current AHPRA RN registration with the Nursing and Midwifery Board of Australia.
- Tertiary qualification in Nursing.
- NDIS Worker Screening Check.
- Right to Work in Australia.
- WWCC (VIC Only).
- Current State Driver's License.
- Computer literacy, including Microsoft Office, Internet, and Database Systems.
- Current required vaccinations.
- Knowledge of the Home Care Package Program.
- 2 to 3 years of experience in Community Nursing.
