We are seeking an experienced and qualified Clinical Care Coordinator to join our team at Signature Care - Residential Aged Care. As a Clinical Care Coordinator, you will play a key role in providing clinical leadership and mentoring to our team of Registered Nurses, Enrolled Nurses, and Personal Care Assistants.

Key Responsibilities
  • Provide clinical leadership and guidance to the care team to ensure high-quality care is delivered to our residents.
  • Drive clinical outcomes and maintain a high level of care through effective care planning and coordination.
  • Be a role model of clinical leadership and professionalism, promoting a positive and supportive work environment.
Requirements
  • At least 2+ years of experience in a Clinical Care Coordinator or similar role in the aged care or acute sector.
  • RN qualification with current unrestricted AHPRA registration.
  • Excellent and up to date clinical knowledge and assessment skills.
  • Experience in staff supervision and development.
  • Strong communication and negotiation skills.
  • A working knowledge of funding and accreditation processes.
  • Knowledge of Care Planning.
