Mercy Aged and Community Care Ltd is seeking a skilled Clinical Care Coordinator to join our team. As a Clinical Care Coordinator, you will play a vital role in coordinating and managing patient care plans, ensuring effective communication between healthcare providers and residents, and monitoring progress to optimize patient outcomes and quality of care.
Key Responsibilities
- Coordinate and manage patient care plans to ensure high-quality care and optimal patient outcomes
- Develop and maintain effective relationships with healthcare providers, residents, and families
- Monitor patient progress and adjust care plans as needed
- Collaborate with multidisciplinary teams to ensure seamless care delivery
- Ensure compliance with relevant laws, regulations, and organizational policies
Requirements
- Strong leadership experience in the health, disability, or home care sectors
- Sound knowledge of aged and disability sector systems and funding, business acumen, and financial literacy
- Excellent time management, organizational, and communication skills
- Compassionate, flexible, and positive approach to effective patient care and management
- Ability to be flexible and adapt to meet clients' individual needs
Preferred Qualifications
- Current NDIS check
- Current COVID-19 certificate and proof of current influenza vaccination
