We are seeking an experienced and motivated Clinical Care Coordinator to join our team at Royal Freemasons LTD.
Key Responsibilities- Provide clinical leadership and work collaboratively with the care team, residents, and families to oversee and guide effective resident-focused clinical outcomes.
- Liaise with relevant Residential care staff, medical, allied health, hospital, and other specialist health practitioners to ensure residents receive high-level clinical care.
- Focusing on clinical service delivery, compliance, and audit processes.
- Care planning in line with new standards.
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) administered through the Australian Health Practitioner Regulation Agency (AHPRA).
- Recent experience in caring for high-care and dementia residents.
- Knowledge of wound care, palliative care, behaviour management, clinical assessments, care plans.
