We are excited to announce an opportunity for a Clinical Care Coordinator to oversee our dedicated care team. This role is pivotal in ensuring that we uphold the highest standards of care for our residents.
Position Summary
As a Clinical Care Coordinator, you will work closely with the Clinical Care Manager and General Manager to maintain compliance with all relevant regulations and guidelines. Your leadership will be essential in fostering a quality lifestyle for our residents while coordinating individual care assessments and evaluations.
Key Responsibilities
- Oversee all clinical services in accordance with established policies and procedures.
- Promote continuous improvement initiatives across clinical and lifestyle services.
- Ensure timely and accurate completion of clinical documentation.
- Coordinate and support scheduled audits and monitor clinical indicators.
- Maintain up-to-date resident health records and clinical documentation.
- Adhere to best practice guidelines for medication management.
Candidate Profile
The ideal candidate will be a collaborative leader with:
- Relevant tertiary qualifications or equivalent recognition in Australia.
- Current AHPRA registration as a registered nurse.
- Extensive senior clinical care experience, particularly in the aged care sector.
- Strong understanding of care assessment processes and AN-ACC.
- Excellent interpersonal and communication skills, both written and verbal.
- Proficiency in various IT applications.
- In-depth knowledge of AN-ACC funding and aged care standards.
