Job Description

The Care Hub is an initiative that brings together organisations to support Children and Young People in areas of Mental Health, AOD, Cultural Connection, Family Work, Community Engagement and Therapeutic Support.

You will join a collaborative team supporting those entering care for the first time.

As a Senior Practitioner, you will undertake assessments, coordinate care plans, and be the point of contact for children/ren and young people during their time in the hub.

You will also coordinate multidisciplinary teams, facilitating assessment, planning, and support for children and families.

  • Managing risk and wellbeing within the Best Interest Framework
  • Understanding how trauma affects young people's development
  • Solid understanding of Child Protection and Welfare system in Victoria
  • Coordinating care team meetings and professional meetings with young people, families, and other agencies
  • Developing plans and assessments, report writing, case noting, recording, and monitoring compliance
  • Providing support to team members and working collaboratively with other teams and agencies
Key Responsibilities
