Young People Mental Health Support Specialist

A unique opportunity to support young people in their recovery journey, providing them with the tools and resources necessary to achieve their goals.
About This ServiceThe Youth Outreach Recovery Service is a psychosocial support model designed for young people aged 16-25 who have been experiencing mental health symptoms for more than six months. It provides eligible young people with short to medium term psychosocial outreach support in the community.
Key Responsibilities- Develop professional relationships with participants, their families, and friends.
- Conduct regular face-to-face visits to connect with participants and monitor progress.
- Assist participants in identifying areas of need using assessment and outcomes measurement tools.
- Encourage linkages into various activities to support goal achievement, including other services.
- Develop individually tailored recovery plans with each participant.
- Provide direct practical support to participants to attain required skills and knowledge.
- Support participants in achieving social inclusion through group activities.
- Help participants who are rough sleeping to identify and maintain suitable accommodation.
- Relevant qualifications or experience in youth mental health issues.
- Current valid driver's license and ability to undertake travel.
- Working with Children Check and NDIS Workers Screening Check.
- Right to Work within Australia.
- Satisfactory National Police Records Check.
- NDIS Worker Orientation Module Certificate.
