Our organisation seeks a dedicated individual to fill this vital role, supporting young people aged 16-25 who are experiencing mental health symptoms.
About The Opportunity- You will work collaboratively with stakeholders to develop and implement our vision, mission and strategic objectives.
- Engage participants, their families and friends in the program where appropriate, developing professional relationships.
- Conduct regular support visits to connect with individuals and monitor progress around key areas.
- Assist participants in identifying areas of need using assessment tools.
- Encourage linkages into various activities to support goal achievement, including other services.
- Develop tailored recovery plans with each participant, incorporating specific goals focused on skill and knowledge development.
- Provide direct support to participants to attain skills required for social inclusion.
- Support participants in securing suitable accommodation.
- Utilise lived experience if in a dedicated peer role, according to the principles of Intentional Peer Support.
