Key Role in Community Care Services

Overview

Our goal is to provide personalized care and support services for people living independently at home. We prioritize building strong relationships with our clients, carers, and families.

This role involves working collaboratively with a dedicated team to deliver high-quality care and support services that meet the unique needs of each individual.

Responsibilities
  • Establishing and maintaining effective relationships with clients, their families, and other stakeholders.
  • Developing, implementing, and reviewing personalized care plans that address client needs and preferences.
  • Conducting assessments to identify client requirements and ensuring compliance with relevant regulations and standards.
  • Providing in-home clinical services, including medication management, wound care, chronic disease management, and palliative care.
  • Acting as a liaison between referral sources, families, and the care team to ensure seamless communication and care delivery.
  • Ensuring adherence to all relevant policies and procedures.
  • Supporting professional growth and development among care team members.
  • Supervising and guiding non-clinical team members to maintain high standards of care.

Working Hours: Monday to Friday, 8:00 am to 4:00 pm, with participation in a rotating on-call roster when required.

Qualifications

Essential:

  • Current registration as a qualified Registered Nurse with AHPRA.
  • Strong organizational skills and proactive approach to problem-solving.
  • Excellent written and verbal communication, with ability to build relationships and achieve positive outcomes.
  • Minimum 2 years' experience in a care management/coordination role within aged care or community care.

Desirable:

  • Minimum 12 months of experience in a similar role is desirable but not essential.
  • Strong interpersonal and communication skills.
  • Above-average time management and organizational skills.
  • Knowledge of aged care and disability sectors.
  • Ability to work autonomously and collaboratively within a team.
  • Proficiency in care management software and computer systems.
