Care Services Coordinator

Description: The role involves supporting complex clients, delivering clinical care services and building strong relationships with clients, carers, and families.
- Developing individualised care plans to meet client needs.
- Conducting assessments to identify client requirements and ensuring compliance with relevant standards.
- Deliver in-home clinical services, such as medication management, wound care, chronic disease management, and palliative care.
- Acting as a liaison between referral sources, families, and the care team to ensure seamless communication and care delivery.
Key Responsibilities:
- Supporting complex Home Care Package clients and delivering high-quality clinical care services.
- Building strong relationships with clients, carers, and families.
- Developing, implementing, and reviewing individualised care plans to meet client needs.
- Conducting assessments to identify client requirements and ensuring compliance with relevant standards.
Required Skills and Qualifications:
- Qualified Registered Nurse with current AHPRA registration.
- Strong organisational skills and a proactive approach to problem-solving.
- Excellent written and verbal communication skills.
