Resident-Focused Clinical Care Specialist

We are seeking an experienced clinical care professional to join our residential aged care facility as a Clinical Care Coordinator. In this role, you will provide clinical leadership and work closely with the care team, residents, and families to oversee resident-focused clinical outcomes.
The ideal candidate will have current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) administered through the Australian Health Practitioner Regulation Agency (AHPRA), recent experience in caring for high care and dementia residents, and knowledge of wound care, palliative care, behaviour management, clinical assessments, and care plans.
As a Clinical Care Coordinator, you will be responsible for undertaking or supervising others to undertake clinical care for individual residents, including complex wound care, pain management, behaviour management, end-of-life care, and other clinical procedures as necessary. You will also liaise with residential care staff, medical, allied health, hospital, and other specialist health practitioners to ensure residents receive high-level clinical care.

About This Role
This is a part-time opportunity, Monday to Thursday. The successful candidate will have the ability to work independently and as part of a team, excellent communication skills, and a patient-focused approach. Requirements
- Current registration as a Registered Nurse with the NMBA
