SACARE is a leading provider of innovative in-home and supported accommodation care solutions to people living with a disability.

About the Role

We are seeking a full-time professional to support individuals with disabilities and their families, ensuring they receive high-quality care tailored to meet their unique needs.

  • As a key member of our team, you will work closely with clients and stakeholders to develop personalized care plans that align with their goals and aspirations.
  • You will be responsible for supporting individuals to navigate the NDIS planning process and ensuring timely and accurate record keeping.
  • Actively participating in team meetings and case conferences is also a key responsibility.
Key Responsibilities
  1. Developing and maintaining accurate and detailed records of client interactions and service delivery.
  2. Ensuring services offered by SACARE are person-centred and tailored to meet individual requirements.
  3. Maintaining up-to-date knowledge of Disability related legislation, policy, and processes.
  4. Demonstrated experience working within the Disability sector, including knowledge of NDIS and DHS standards.
  5. Excellent administrative, time management, and organisational skills.
Requirements
  • Qualifications in Community Services or relevant demonstrated experience.
