Extended Hours Clinical Care Coordinator

We are seeking a highly skilled and compassionate professional to deliver evidence-based clinical care to adults aged 18 years and over who experience mental health concerns.

  • This role involves the delivery of high-quality clinical services, including intake and assessments, risk assessments, care treatment planning, and provision of brief psychological intervention.
Key Responsibilities:
  • Conduct comprehensive assessments to identify individual needs and develop personalized care plans.
  • Collaborate with multidisciplinary teams to provide coordinated care and support services.
  • Provide education and support to individuals and their families on mental health conditions, treatment options, and recovery strategies.
  • Maintain accurate and up-to-date records of client interactions and progress.
Requirements:
  • AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist, or AASW registration/eligibility as a Social Worker.
  • Experience in working with adults with mental health concerns and their families.
  • Strong communication and interpersonal skills, with the ability to engage with diverse individuals and communities.
  • Ability to work flexibly, including afternoons, evenings, weekends, and public holidays.
