Job Title: Skilled Aged Care Nurse

Western District Health Service is seeking an experienced and dedicated Skilled Aged Care Nurse to join our team of healthcare professionals. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality patient care and support to our patients and their families.

Key Responsibilities:
  • Deliver exceptional clinical care to patients in our aged care facilities, ensuring their physical, emotional, and social needs are met.
  • Develop and maintain strong relationships with patients, families, and colleagues through effective communication and teamwork.
  • Participate in ongoing professional development opportunities to enhance your skills and knowledge in aged care practices.

Requirements:
  • Current registration as a Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Strong clinical skills and knowledge of aged care practices, including dementia care and palliative care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality patient outcomes.
