Job Description

We are a leading provider of support services to individuals with complex needs, working in partnership with them and their families to empower independence and achieve goals. As part of our multidisciplinary team, you will be supporting NDIS participants in the Bunbury region to live more independently.

Key Responsibilities
  1. Supporting NDIS participants and their families to identify needs through assessment and person-centred planning
  2. Evaluating, developing and delivering evidence-based support plans within a complex support environment
  3. Developing meaningful goals and practical strategies aligned with the positive behaviour support framework
  4. Ensuring individual plans reflect participants' goals, passions and strengths
  5. Building knowledge, capacity and strengths of those who support participants
Requirements

To be successful in this role, you will require:

  • Current suitability at Core, Proficient or Advanced level as a Behaviour Support Practitioner
  • Considerable experience providing support to individuals with behaviours of concern
  • Completed a degree-level qualification as an allied health professional (desired)
  • Excellent time management and organisational skills
  • A proactive attitude and genuine passion for working in the disability sector
