Our ideal candidate will be responsible for coordinating the delivery of care services to clients with complex needs. This role requires effective communication, problem-solving skills, and the ability to manage multiple tasks simultaneously.

The Case Coordinator will work closely with healthcare providers to ensure that clients receive the necessary support and services. They will also be responsible for monitoring and evaluating the effectiveness of care plans, making adjustments as needed.

About the Role

  • Manage icare's relationship with the client and dependents throughout the life of the case.
  • Develop, monitor, and evaluate care plans, making adjustments as needed.
  • Maintain accurate information and case management records.
  • Liaise with clients and families to obtain information and results.
  • Deal with multiple health jurisdictions, hospital/laboratory medical records, privacy, and freedom of information policies and procedures.

Required Skills and Qualifications

  • Highly developed communications skills.
  • Ability to solve problems and manage multiple tasks with conflicting deadlines and demands.
  • Critical reasoning skills to ensure a high degree of integrity in decision-making processes.
  • A well-developed understanding of the needs and perspectives of people with a serious illness.
