This specialist will provide person-centred support to clients with mental health and wellbeing challenges, empowering them to identify their goals and take steps toward recovery.
About Our ServiceOur Saint James Transitional Care Unit supports adults aged 18–64 to transition from hospital to community or prevent hospital admissions.
The service is delivered by a multidisciplinary team of mental health clinicians and peer workers, helping clients achieve their goals, build life skills, strengthen relationships, connect with support networks, and develop the confidence and capacity to live independently.
