About the Role:
  • Provide social, emotional, and practical support to individuals on an individual and group basis.
  • Assist with identifying needs for wellness, options for recovery, and support to navigate mental health services towards discharge.
  • Promote hope for recovery, choice, and self-determination, using shared experiential knowledge, skills, and strategies for living with mental health issues and recovery.

The Complex Needs Coordination Team has multiple bases across Western Australia. The successful applicant will be part of a compassionate, inclusive, and mission-driven team, working with individuals to promote recovery and well-being.

Key Responsibilities:
  1. Work collaboratively with the multi-disciplinary team to provide comprehensive support services.
  2. Conduct assessments and develop plans to meet individual consumer needs.
  3. Build strong relationships with consumers, families, and carers to ensure effective support and care.
