Job Title: Lifestyle Coordinator

We are seeking a highly motivated and caring individual to join our team as a Lifestyle Coordinator. As a key member of our care team, you will play a vital role in enhancing the lives of our residents by creating personalized programs that cater to their physical, intellectual, emotional, cultural, and social well-being.

Main Responsibilities:
  • Develop lifestyle programs based on individual needs, preferences, and goals, ensuring a high level of engagement and participation from residents.
  • Lead and participate in delivering engaging activities that cater to diverse interests, promoting social interaction and community building.
  • Conduct thorough and accurate lifestyle assessments of residents, identifying areas for improvement and developing strategies to address these needs.
  • Ensure complete and timely documentation of resident participation and engagement, providing valuable insights for future program development.
  • Mentor and support the lifestyle team to deliver exceptional lifestyle experiences, promoting teamwork and collaboration.
  • Foster strong relationships with residents, families, and volunteers, ensuring a warm and welcoming environment.
  • Cultivate opportunities for residents to engage with the broader community, promoting social inclusion and connection.
Requirements:
  • Demonstrated understanding of leisure and lifestyle programming, including Aged Care Quality Standards.
  • Certificate IV in Leisure and Health or equivalent qualification.
  • Previous experience in a similar role, including dementia care, with a proven track record of success.
  • Experience using computerized care management systems and excellent computer literacy skills.
  • Strong written and verbal communication, interpersonal, and leadership skills.
