Independent Living Support Specialist

Supporting individuals with disabilities to live independently and pursue their passions is at the heart of this role.
About the Position- You will provide person-centred support across various settings, fostering meaningful relationships and empowering participants to make informed decisions about their lives.
- Assist with daily living activities and personal care in home and community settings, promoting independence and dignity.
- Deliver tailored support plans in collaboration with a multidisciplinary team, ensuring individualised care and attention to detail.
- Share your interests and hobbies with participants, enriching their experiences and creating opportunities for social inclusion and community participation.
- Work closely with colleagues to ensure seamless delivery of services, prioritising participant needs and safety above all else.
- Support participants in developing life skills and achieving their goals, celebrating successes and learning from setbacks.
- Contribute to a culture of continuous improvement, sharing knowledge and best practices with colleagues to enhance the quality of service delivery.
