Job Description:

We are seeking a skilled and compassionate Healthcare Specialist to join our team. As a key member of our healthcare team, you will be responsible for providing exceptional care to our clients. Your expertise in nursing services will enable you to effectively coordinate, implement, and evaluate patient care.

About the Role:

The successful candidate will possess strong clinical skills and experience in triage, client assessment, and care planning. You will work closely with allied health professionals, community care services, and specialist practices to provide comprehensive care to patients. Your excellent communication skills will enable you to facilitate effective communication between patients and healthcare providers.

Responsibilities:

  • Triage, client assessment, and care planning
  • Clinical data management
  • Liaising with allied health professionals, community care services, and specialist practices
  • Facilitating effective communication between patients and healthcare providers
  • Patient advocacy
  • Management of human and material resources
  • Optimising the use of professional resources
  • Building practice capacity to adapt to change

Required Skills and Qualifications:

  • Registered Nurse with current registration with Australian Health Practitioner Regulation Agency (AHPRA)
  • At least 2 years' full time equivalent as a Registered Nurse in an Acute, Community Care and / or General Practice setting
  • Excellent knowledge and understanding of working with clients to achieve social, emotional and physical wellbeing
  • Superior patient care skills that reflect a professional but friendly approach to liaise effectively with a diverse range of people
  • Ability to analyse and utilise clinical data in service development and quality improvement initiatives
  • Highly motivated with excellent interpersonal and communication skills so has the ability to work effectively in a team or independently
