Key Responsibilities- Organise a Clinic for Dementia and Aged Care alongside a Rehabilitation Specialist
- Provide vital support through home visits
- Contribute to person- and family-centred care within a multidisciplinary team
The ideal candidate will have current registration with AHPRA as a Registered Nurse, proven clinical experience in dementia, delirium, or aged care case management, excellent communication, assessment, and time management skills.
