Clinical Lead

We are seeking an experienced and strategic Clinical Lead to oversee our After-Hours Health Outreach Service (AHOS), which provides critical health interventions to adults with complex needs in Melbourne's CBD.
This role manages AHOS, our street-based outreach nursing program (operating Wednesday to Sunday nights) and non-medical clinical staff based at The Living Room, Melbourne's renowned Primary Health Service for people experiencing homelessness.
Key Responsibilities- Drive the delivery of high-quality, client-centred care while enhancing access to multidisciplinary health services during after-hours periods.
- Strengthen the systems and processes that support coordinated care between clinic and outreach-based services.
- Provide clinical leadership, coordinate daily operations, and ensure the professional development and clinical supervision of the nursing team.
- Take a lead in clinical governance, foster collaborative partnerships with external health, outreach, and housing services, and guide service integration within a multidisciplinary framework.
- Demonstrated experience (2+ years) in a community health setting.
- Demonstrated experience (2+ years) in delivering clinical interventions to disadvantaged populations.
- Registered Nurse with AHPRA Registration.
- Experience in developing partnerships that deliver improved referral pathways and outcomes for clients.
- Demonstrated experience in leading a team of client-focused, resilient, and high-performing staff.
