Job Title:

Aged Care Clinical Assessor

Description:

We are seeking a skilled and compassionate individual to join our team as an Aged Care Clinical Assessor. In this role, you will be responsible for conducting thorough assessments of older adults to identify their needs and connecting them with services that support independent living.

Your responsibilities will include:

  • Conducting comprehensive assessments to identify the needs of older adults
  • Facilitating referrals to essential services that support independent living
  • Promoting function and capability in daily living
  • Working closely with families and carers, recognizing them as key stakeholders
  • Ensuring all assessments are conducted in line with Department of Health contractual requirements
Requirements:
  • At least 1 year of experience in aged care, including client assessments and/or service delivery
  • Tertiary education in a health-related discipline (e.g., nursing, occupational therapy, physiotherapy, social work)
  • Current unrestricted General Registration with AHPRA or relevant professional association
  • Proficiency in Word, Excel, Outlook, and client management systems
  • Current Australian driver's licence, comprehensively insured vehicle, and the ability to travel within your service area
