Job Summary

The Monash Health Community seeks a dedicated and experienced Clinician to join our dynamic team as a Post Acute Care Coordinator. This is an exciting opportunity for a like-minded professional to work collaboratively with our multi-disciplinary team to enhance the client's journey through non-acute care.

We are seeking a Nursing or Allied Health Professional with relevant qualifications, experience working in acute or community settings, and a strong commitment to delivering patient-centered care and advocacy.

About the Role

This permanent position involves coordinating services for referred clients, ensuring timely and seamless allocation to clinicians. Key responsibilities include:

  • Triage and registration of clients
  • Allocation of services to clinicians
  • Collaboration with multi-disciplinary teams

About You

We are looking for a flexible and dynamic professional who is committed to safe best practice and quality care. The ideal candidate will have:

  • Relevant qualifications in Allied Health or Nursing Related Field
  • Current Victorian Driver's Licence
  • Demonstrated recent experience working in an acute or community setting
  • Minimum 5 years post graduate experience
  • AHPRA registration or registration with National Registration Authority
  • Australian working rights
