This position sits within the **Health Independence Program (HIP) **suite of services that are delivered across St Vincent's Health Melbourne (SVHM) and North Richmond Community Health (NRCH). Post Acute Care is funded by Department of Health and assists clients to recuperate at home following discharge from a public hospital.
**About the Opportunity**
NRCH has an exciting opportunity for a registered nurse or registered allied health clinician. The **Care Coordinator (fixed-term 0.8-1.0 EFT) **will provide support to clients in the post discharge period and assist them navigate the service system.
The PAC program has a flexible service delivery model enabling the program to provide a responsive service which includes purchasing a wide range of health and community services and provision of clinical care by program staff when necessary.
**More specifically, some of your responsibilities will include**:
- Intake, screening and assessment;
- Liaising with clients, their carers/families, hospital clinicians and other community service providers;
- Developing care plans with clients, engaging / contracting a range of clinical and in home support services;
- Provision of clinical services when required
- Developing and maintaining strong networks with hospital clinicians, community service providers and neighbouring other Health Independence Programs
**Key Selection Criteria**:
- AHPRA registration - registered nurse or allied health clinician with significant experience working within the health and/or community services sector.
- Experience working with people with complex issues in a health setting (including homelessness, drug & alcohol, mental health, dementia, refugee and migrant communities), and the proven ability to work with people from CALD (culturally and linguistically diverse) backgrounds.
- an enthusiastic, highly organised team player with a strong knowledge of the community services available to clients in the recuperative phase.
- To ensure the smooth provision of services to clients, you will have demonstrated ability to identify issues, problem solve, be an effective communicator with a wide range of stakeholders and meet deadlines in a fast-paced environment.
- You must have excellent computer skills including MS Office suite.
- Current Victorian Driver's Licence
Please note it is a requirement for all employees of North Richmond Community Health to demonstrate evidence of mandatory immunisations/vaccinations prior to commencement of employment, based on the Immunisation Category this position falls under this includes 3 doses COVID-19 vaccine and the annual influenza vaccine.
