Role Overview

A dynamic opportunity exists for a Dementia Support Coach to join our team on a fixed term full-time contract. Our team is dedicated to providing high-quality care and support to individuals living with dementia and their families.

As a Dementia Support Coach, you will facilitate on-the-ground support, implementation of relationship-based support strategies, modelling and coaching with clients, carers and care staff.

Key Responsibilities:

  • Collaborate with peers to develop the skills and expertise of aged care staff and carers to understand the person living with dementia and provide best practice support and care.">
  • Support the implementation of recommendation plans on the ground, utilising modelling and coaching techniques, and seek guidance as required.">
  • Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance as required.">

Essential Skills & Qualifications:

We are seeking an individual with strong organisational abilities, including effective time management and prioritisation in a busy work environment. Other requirements include:

  • The ability to work independently and as part of a team.">
  • Demonstrated commitment to excellence in customer service principles, including conflict resolution, to ensure consistent provision of exceptional service.">
  • A current driver's licence and safe driving record.">
  • Willingness to travel and work intrastate and interstate for up to five days at a time to support the delivery of services.">
  • Flexibility to work hours as required to meet client needs.">
