Job Description

Our Support Coordinators play a vital role in helping NDIS participants implement their plans, build their capacity and navigate complex service systems.

The key responsibilities of this role include:

  • Providing personalised and goal-oriented support coordination to NDIS participants
  • Assisting participants in understanding and activating their NDIS plans
  • Connecting participants to mainstream, informal and funded supports
  • Monitoring progress toward goals and ensuring service delivery aligns with participant needs
  • Building participant capacity for self-advocacy and independence
  • Coordinating multidisciplinary support and resolving service barriers
  • Preparing case notes, progress reports, and maintaining all required documentation
  • Collaborating with internal teams and external stakeholders to provide integrated care
  • Advocating for participants' rights and promoting informed decision-making
Required Skills and Qualifications

To be successful in this role, you will need:

  • A minimum of 5 years of experience in Support Coordination, case management, or allied health roles
  • In-depth knowledge of the NDIS framework, funding streams and compliance
  • Proven ability to support individuals with complex needs, including disability, psychosocial issues, or dual diagnoses
  • Strong communication, advocacy and interpersonal skills
  • Excellent time management and organisational skills
  • Capacity to work both independently and within a team
  • High level of proficiency in written documentation and digital record keeping
  • A commitment to person-centred, trauma-informed and culturally responsive practice
Qualifications and Requirements

You will need:

  • A relevant qualification background in Social Work/Community Service/Psychology/Allied Health
  • An understanding of the NDIS framework
  • Strong communication and problem-solving skills
  • Demonstrated cultural sensitivity for effective support coordination
  • A current NDIS Worker Screening Check and Working with Children Check (or willingness to obtain)
  • A valid driver's license and access to a reliable, insured vehicle
  • Proficiency in Microsoft Office and case management systems
