Delivering exceptional care coordination is at the heart of this role as an NDIS Clinical Customer Care Manager.

You will be working with customers who receive funded supports via the NDIS, playing a vital part in providing support for Australians with disability, their families and carers.

Key Responsibilities
  • Ensure customer outcomes are met as outlined in their NDIS plan.
  • Assess and sign off care competencies/nursing competencies provided by support workers.
  • Cultivate strong relationships to negotiate and coordinate supports and services that foster independence.
  • Document and deliver required services as per the participant's plan, ensuring seamless delivery.
  • Implement plans and supports through a person-centred approach.
  • Manage Government contractual requirements under the NDIS guidelines.
  • Collaborate with the clinical manager on clinical matters within scope.
Requirements
  • A dedicated individual passionate about the Healthcare Industry, particularly supporting people with disabilities.
  • Proven experience in delivering specialized coordination and assessments within current qualifications.
  • Knowledge in Dual Diagnosis, Mental Health, Youth or Family Support.
  • High emotional intelligence to empathize with customers and adapt to their evolving needs.
  • A positive attitude towards people with disabilities, mental illness, and their families and friends.
  • Current or willingness to obtain necessary checks: police check, working with children check, and NDIS worker screening.
  • 1-2 years of community nurse experience (essential).
  • APHRA Registered Nurse registration (essential).
