Purpose of the Role

The Service Coordinator is responsible for coordinating services and leading practice at a site level. This involves providing supervision, coaching, and support to staff, as well as supports to clients in the areas of health.

About the Role

  • Coordinating and overseeing the delivery of clinical services to participants, ensuring their needs and goals are met effectively.
  • Collaborating with participants, their families, support teams, and allied health professionals to develop individualized care plans that align with NDIS goals and guidelines.
  • Conducting comprehensive assessments of participants' clinical needs and developing appropriate service plans accordingly.
  • Liaising with healthcare professionals, therapists, and service providers to ensure seamless coordination and integration of clinical services.
  • Maintaining accurate and up-to-date documentation, including progress notes, incident reports, and service records, in accordance with NDIS requirements.

About You

  • A bachelor's degree in a relevant field such as nursing, social work, psychology, occupational therapy, or a related discipline.
  • Demonstrated experience and knowledge in providing clinical services, preferably in the context of disability support and the NDIS.
  • Demonstrated understanding of NDIS, principles of Active support, and human rights-based approach to service delivery.
  • Strong interpersonal and communication skills.
  • Experience managing and supervising staff and client outcomes in multiple accommodation settings.
