About the role

This is an exciting opportunity to join the BETTER SUPPORT SOLUTIONS' team as an NDIS Service Coordinator (RN) in our Parramatta, NSW office.

You will play a crucial part in providing high-quality care and support to our clients with disabilities, helping them achieve their goals and live more independent lives.

Full-time and part-time positions available – choose the hours that work for you.

What you'll be doing

  • Manage daily operations across participants
  • Preparing in-home / SIL care assessments and drafting care plans to best meet client requirements.
  • Ensure care aligns with NDIS goals and practice standards and client matching.
  • Lead and mentor support staff to work effectively with complex needs
  • Maintain rosters and manage workforce performance

  • Uphold compliance with restrictive practices and Quality & Safeguards Commission guidelines

  • Mentor and support suppport workers of the team

  • Drive performance reviews and staff development plans

What we're looking for

  • Current registration as a Registered Nurse (Division 1) with AHPRA with a strong understanding of the NDIS and disability support services
  • At least 2 year experience as a scheduler, case manager, care coordinator, hospital discharge planner, or NDIS service coordinator
  • Excellent communication and interpersonal skills, with the ability to build trust and collaborate effectively with participants.
  • Strong problem-solving and decision-making skills, with a focus on achieving positive outcomes for participant
  • Proficient in managing complex caseloads and maintaining accurate records and documentation
  • Commitment to delivering high-quality, person-centred care and support
  • Willingness to travel within the local area to visit participants in their homes or the community
